IMPORTANT REMINDERS
- The open enrollment window period is the time during which you may change medical and/or dental plans and add dependents to the benefit plans. Please keep in mind that dependent children are eligible to age 19, or to age 25 if they are full-time students or financially dependent on you per IRS guidelines.
- You should review the Section 125 options offered through American Fidelity. You must re-enroll to continue your medical and dependent care reimbursement accounts for the new benefit year (January 1 – December 31, 2010). To schedule a meeting, contact your site secretary.
- Any benefit changes will be effective January 1, 2010, and will remain in effect through December 31, 2010.
ATTENTION DELTA DENTAL Plan MEMBERS: If you terminate Delta Dental DPO coverage for an eligible dependent, they may not be re-enrolled in the Delta Plan at a later date unless they have lost coverage under another plan (proof of loss of coverage is required) or have re-qualified as a dependent. Enrollment must be within 30 days of the qualifying event.
